What to expect

When we receive your enquiry, we’ll ask you to complete a short questionnaire.
This makes sure our service is right for you and tailored to your needs.

If you have been diagnosed with dementia

We can review your current treatment plan, including your memory medication, and improve it wherever possible.

We also provide cognitive stimulation therapy (CST), which is recommended by the latest NICE guidelines and has positive outcomes for cognitive functioning, social interaction, communication and quality of life. CST is as effective as memory medication.

2. Questionnaire

You complete a short questionnaire about your memory and health, reviewed by our consultants.

3. Initial assessment

Up to 60 minutes, face to face or by video, covering your history and a comprehensive memory test.

4. Diagnosis and plan

We explain your results, make a diagnosis where applicable, agree a treatment plan and send a report to your GP.

It has taken a long time for the diagnosis it was disregarded, overlooked by the NHS medical and nursing staff. The NHS missed vital evidence ie the MRI when mentioned it was ignored.

He goes out of his way to clearly explain procedures and how the new medication may produce side effects. He organised the MRI scan with a local centre and produced a full report afterwards for both my wife and her local GP. This has given the basis for us to contact Dementia Forward and look into the possibility of additional help as the condition progresses. We immediately felt listened to and were appreciative of his kind, friendly but professional manner. He gave us time and carried out necessary assessment sensitively.  He remained thoughtful in treatment and diagnosis of my husband's Alzheimer's and monitored the medication. Care has now been handed over to our local GP but Dr Sajjadi has assured us of his availability and the possibility of future consultations when requested. Clear, fixed fees

Assessment and diagnosis

£990

Two consultations with a consultant memory specialist for assessment, diagnosis and report. Includes an eight-week private prescription for memory medication if applicable.

Single extended appointment

£800

If you already have your blood tests and brain scan, a single long appointment can be offered in most cases instead of two.

Follow-up consultation

£225

Up to 30 minutes with a memory specialist consultant to review your progress and medication.

Private blood tests and brain scans are charged separately by the provider. An MRI head scan is generally around £400, and some NHS GPs are happy to arrange scans via the NHS.
